About the Role
An attractive position is currently available for an experienced allied health or nursing professional to extend skillset and work as a Case Manager with the Transition Care Program based at St George's Health Service in Kew.
The Transition CareCase Manager assumes a key role in the co-ordination and case management of patients within the Transition Care Program and works closely with multidisciplinary and specialised teams through the transition episode of care.
Duties
Direct clinical provision of high quality and evidence-based patient centred care.
Undertake holistic comprehensive assessments in collaboration with the client using case management priciples and practice frameworks including strengths based approaches.
Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
Facilitation of client generated goals that are realistic, time limited and based on addressing health and wellbeing needs
The classification ranges for this position are:
Allied Health Grade 2:
Social Worker Gr 2 SC21 - SC24 ($86,730 - $99,699 per annum, pro rata)
Occupational Therapist Gr 2 VF6 - VF9 ($86,730 - $99,699 per annum, pro rata) or,
Registered Nurse Grade 4A YW7 ($107,962 per annum, pro rata)
Your Contribution
Tertiary Allied Health or Registered Nurse qualification
Minimum of 5 years' experience in a health care setting
Demonstrated ability to work autonomously and as part of an interdisciplinary team
Demonstrated ability to effectively work with relevant service providers to achieve best outcomes for clients
Proven ability to effectively manage and negotiate solutions to complex problems
Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organisation, planning and problem solving
Current national criminal history check, or willing to obtain

About Transition Care Program
Transition Care services target older people at the conclusion of a hospital admission who require more time and support to complete their restorative process, optimise their functional capacity and finalise and access their longer term care arrangements.
The team includes Case Managers, physiotherapists, occupational therapists, podiatrists, dietician, speech pathology, allied health assistants and administrative support.
