- Salary: $86,950 - $100,729 (pro rata) - AHP2; $82,398 - $104,154 (pro rata) - RN/RM2C
- Perm P/T position (18.75 hrs p/wk if appointed at AHP classification/19 hrs p/wk if appointed at RN2 classification)
Please note that this position will be filled at either AHP-2 or RN-2C classification.
Applying your clinical experience, specialist clinical knowledge and professional competence you will plan, implement and evaluate a comprehensive and integrated range of services appropriate to the needs of the South East community. You will work under limited clinical supervision and may provide direction to less experienced allied health or nursing staff and students. As a member of a multidisciplinary team, including health professionals and service providers from other sectors, you will utilise a combination of preventative, early intervention, treatment/therapy and evaluation approaches. You will be responsible for the effective and efficient implementation of the Transitional Care Package (TCP) Program in the South East region and surrounding areas. In accordance with the TCP Guidelines, you will ensure recipients receive quality rehabilitation and restoration focused services which are appropriate to their needs and well-coordinated through effective case management. You will also contribute to network-wide consistency of the Transitional Care Program by ensuring all administrative, reporting and evaluation requirements are met.
For appointment at the AHP2 level, you must hold a recognised qualification within a relevant Allied Health profession and be eligible for practicing membership of a relevant professional association. For appointment at the RN/RM2C level, you will be eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold a current practicing certificate. Demonstrating the ability to work effectively within a multidisciplinary team, you will be capable of prioritising workloads and meeting set timelines whilst working under minimal supervision.
