About the role
Under clinical direction from the Transition Care Program (TCP) Manager, the AHP2 Occupational Therapist will contribute to the planning, implementation and evaluation of a comprehensive and integrated range of services, appropriate to the needs of the TCP client group. To achieve this, the Occupational Therapist works as a member of a multi-disciplinary team, including health professionals and service providers from acute, sub-acute and community sectors and utilises a combination of preventative, early intervention and
treatment/therapy based approaches.
The Transition Care Team is responsible for the delivery of services to meet the needs of older people with complex health and care needs as they transition from hospital back to the community. The TCP Occupational Therapist will respond to the individual needs of the clients on the program as well as supporting the co-ordinators and Service Providers.
The TCP Occupational Therapist is required to:
- Respond to referrals from TCP co-ordinators in a timely and appropriate manner.
- Provide support and advice to TCP coordinators and external service providers regarding complex matters of discharge planning, decision making, risk management and safety of the client.
- Participate in quality initiatives including the development of policies and procedures, frameworks and guidelines, education and research concerning Occupational Therapy.
An employee at this level accepts accountability for their own practice standards, activities delegated to
others and the guidance and support of less experienced staff.
Qualifications
- Must hold an appropriate degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
- Current Registration with the Australian Health Practitioner's Agency as an Occupational Therapist.
