We have an exciting opportunity for an Assessor in the Veterans’ Home Care based in Blackburn. You will be part of our supportive team in Veterans’ Home Care in a part-time or full-time role.
You’ll have the chance to work independently whilst being part of a trusted team of people, who provide high quality support to eligible members of the Veteran community whilst adhering to the Department of Veterans Affairs VHC Guidelines. You’ll also play a crucial role in enhancing our Veteran's experience through empathy, warmth, understanding, and kindness. You’ll engage in meaningful work where you have the opportunity to provide exceptional customer service.

Your day to day activities include:
- Understand the VHC program parameters and be able to communicate this to client’s families, service providers, and a range of health professionals
- Manage inbound and outbound calls from Veterans, and their families/representatives, service providers, and the Department of Veterans Affairs
- Conduct assessments and reviews to identify any special needs, specific goals, or desired outcomes to maximise independence
- Recommend appropriate VHC services and recommend other primary health, allied health or aged and community care services
- Accurately document all communication in DVA approved program
To thrive in the role you will have:
- Experience in assessment, care planning and service coordination
- Experience in the aged or community care sector of 2 years minimum
- Experience in working as a Personal Care Assistant (PCA) or Personal Care Worker (PCW)
- Experience in customer service, including a professional and courteous manner, and a collaborative ‘can do’ approach (desirable)
- Knowledge of the veteran community and the impact of military training/experiences on the client’s physical and mental health (desirable)
- Proficient, clear, and effective written and verbal English communication skills
- Use technology confidently, including Microsoft Office programs
