$71,132 to $76,271 p.a. + 11% super

ABOUT THE ROLE?
The Support Coordinator – VHC will report to the Coordinator – Veterans Home Care. The role of Support Coordinator exists to implement and coordinate services which effectively respond to the expressed needs and wishes of individuals receiving supports from the Department of Veterans’ Affairs – Veterans Home Care Program
REQUIREMENTS:
Minimum 2 years’ experience in a similar role would be ideal;
Experience working with people with disability, families and carers to provide individualised supports;
Demonstrated commitment to working with people in an encouraging and supportive manner according to the principles of Person Centred Planning;
Demonstrated ability to work effectively with a culturally diverse client group;
Ability to work co-operatively as a member of a professional team;
Demonstrate competency in the use of Client Management systems;
Preferably a tertiary qualification in relevant health/social sciences/disability studies.
Understanding and commitment to implementing Consumer Directed Care Initiatives and person centred approaches;
Current unrestricted Victorian Drivers licence.
NDIS Workers Screening Check;
Must have been vaccinated against COVID-19 in line with the Victorian Government Public Health Directions.

Capacity to negotiate for supports and/or services that are identified in the planning process by the person;
Ability to work flexibly and adapt as the service evolves and responds to the needs of individuals over time;
Ability to commence Immediately.
