Are you a Wellbeing mentor and/or have a lived experience of mental illness and a passion to use this experience in supporting others to create linkages within their community and build capacity?
Can you apply a person centred approach to support a variety of participants requiring psychosocial support?Additional attributes we are looking for include:
- Ability to work autonomously and capacity to contribute positively to a team and share knowledge and resources
- The ability to draw on own lived or Living Experience
- Knowledge of local area services to build a participants support network
- Adaptable to change that occurs within the community services sector
- Effective communication with rapport building skills
You will oversee a portfolio of participants where you will provide short-term case management by providing outreach as well as using your technical skills to implement client centred approach, where no two days are the same. All work is done alongside the service user as our goal is to enable rather than do, to promote independence, development and confidence of the individuals. Our success will enable them to have the voice and choice to thrive in their local communities.The role entails:
- Providing case management and assertive outreach to support participants and their families in their own home and community
- Providing short term assistance to enable participants to work towards their goals
- Assisting to build capacity and working towards outcomes identified by the participant
- Developing and maintaining professional relationships with participants and stakeholders
- Providing accurate and timely case notes including risk management and recovery plans
