The focus of these services is assisting older people to regain or maintain physical, functional and cognitive abilities which support them to either maintain or recover a level of independence, allowing them to remain living in the community. This position works as a member of a Chronic and Complex multidisciplinary team. - Demonstrated understanding of the health and associated care needs of persons with complex, Chronic and/or aged conditions residing in the community.
- Able to utilise relevant evidence based, best practice guidelines to guide decision making for best client outcomes.
- High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases.
- Ability to work independently and as part of multidisciplinary team.
- Current Drivers Licence valid within Australia.
