What you'll be doing
Chronic and Complex (CAC) Allied Health services focus on restoring, improving, or maintaining older people’s independent functioning and wellbeing. The focus of these services is assisting older people to regain or maintain physical, functional and cognitive abilities which support them to either maintain or recover a level of independence, allowing them to remain living in the community. This position works as a member of a Chronic and Complex multidisciplinary team. Responsible for contributing to the planning, implementation, evaluation improvement & reporting of the dietetics service within the Chronic and Complex team

Essential Requirements:
Qualifications: Tertiary qualifications in Nutrition and Dietetics, acceptable to the membership of the Dietitians’ Association of Australia (DAA).
Selection Criteria:
- Demonstrated skills in Dietetic assessment, planning and evaluating interventions for clients with Chronic and Complex health conditions.
- Demonstrated understanding of the health and associated care needs of persons with complex, Chronic and/or aged conditions residing in the community.
- Able to utilise relevant evidence based, best practice guidelines to guide decision making for best client outcomes.
- High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases.
- Ability to work independently and as part of multidisciplinary team.
- Current Drivers Licence valid within Australia.
