About the Role
- Provide clinical services to patient/client/consumer groups and circumstances of a complex nature requiring advanced practice skills and operate independently with minimal direct supervision.
- The Young ParentsSocial Worker works with Young Parents up to the age of 24 years.
- Provide supportive case management for up to two years and is a home visit-based program.
- Clinical supervision will be provided as per the AASW and HNELHD guidelines.
- The successful applicant will be based at Muswellbrook Community Health as part of the Upper Hunter Social Work team and will work closely with other HNELHD services and services based in the local community.
- Muswellbrook is located in the rolling vineyards of the world-famous Hunter Valley wine country, where you can taste incredible local produce, take beautiful scenic drives, and discover art galleries featuring acclaimed national works.
- Muswellbrook is less than a 3-hour drive from Sydney, and less than 2 hours from Newcastle.

- You will be supported by the Upper Hunter Social Work team which comprises of a small team of Social Workers who are located together at “the Cottage” on the Muswellbrook Health service site.
- The successful applicant will be line-managed by the Social Work Head of Discipline who is also based at the Muswellbrook Health service campus.
- You will have access to relevant training, education and support relevant to their role and will be well supported by the Social Work team, and the wider Muswellbrook community health team.

- To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia.
