headspace provides support to young people aged 12-25 experiencing mild to moderate or emerging mental health issues. The multidisciplinary team at headspace Canberra work to improve health, social and vocational/educational outcomes for young people. Grand Pacific Health is the lead agency for headspace Canberra.
Grand Pacific Health is a not-for-profit, primary health care organisation. We’re here to provide our local communities in NSW and the ACT with equitable, inclusive and integrated health care including physical health and mental health care services. We currently employ 340 people, provide services from 25 locations and are continuing to grow.
About the position
The Youth Care Coordinator will work collaboratively with a young person and their family/carers to assess the mental health and psycho-social support needs of the young people referred to headspace.
They will coordinate the transition to appropriate support and treatment streams within and external to headspace in accordance with the young person’s clinical need.
The position will also have a community engagement component, working closely with the community engagement officer for health promotion and related activities.
We are looking for someone with:
- Qualification in or working towards same in youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other relevant field or demonstrated experience in a similar role.
- Demonstrated understanding of issues affecting the health and wellbeing needs of young people with mental health and/or substance use problems.
- Experience working within a team environment, providing coordination of care and ability to work from a recovery-oriented framework.
- Passion and experience working with young people
