We provide a wide range of services, activities and programs across 5 Services – Children's Services, Youth Services, Multicultural Communities, Aged and Disability Care (ADC) and Community Engagement.. The role primarily focuses on providing Early Intervention Case Work and support to young people, predominantly in the 12-18 age range, who have complex needs, who are homeless or at risk of homelessness across the South Western Sydney Area District.

The position will provide this support in young people’s environments, to improve their access to homelessness services and support initiatives, education & training, living skills training and links to broader health and mental health, welfare, child protection and alcohol and other drug services.

The position will liaise with and coordinate support from agencies that offer relevant services to the clients group and their families/ carer givers.

 The position requires a commitment to organizational values and the achievement of strategic and business objectives. All work undertaken will reflect CORE Community Services Mission and Values. 

  The role will require the incumbent to initiate new community partners to establish outreach and training programs.

Key accountabilities of the role are:

Undertake and meet all case management responsibilities in relation to young people allocated by the Senior Case Worker. Casework with a manageable caseload

Maintain regular, effective and meaningful contact with young people.

Work within a client-centred, trauma-informed framework that is culturally appropriate.

 Ensure that case plans are regularly reviewed and facilitate the attendance of all key stakeholders at relevant meetings including case conferences and reviews.

 Attend regular meetings from each service area in Youth Services to provide culturally appropriate services for clients, and gauge culturally appropriate training needs. Identify personal learning needs and give feedback on the service including the identification of gaps and areas for improvement.

 Provide clear and concise advice, assistance and/ or recommendation strategies to Young People and their families in South West Sydney

Actively marketing and promoting CORE Youth Services programs and activities to new prospective participants and service providers. 

Develop and maintain positive and effective professional working relationship with internal and external stakeholders.

Work collaboratively with Team Leaders, Case Managers, Residential Care Workers, administration staff and other Core Community Service staff to achieve identified outcomes for young people.

Undertake case management including reviews, reporting and administration functions.

Work collaboratively with staff to facilitate group work activities in line with the Targeted Early Intervention (TEI) Outcomes Framework.

Selection Criteria:

  • Tertiary qualifications or higher in Social Work, Social Science, Community Welfare or community sector or equivalent and 2 years’ minimum experience.
  • Demonstrated extensive experience working with vulnerable young people and young families aged 16-24 with complex needs from a variety of backgrounds and understanding of the issues they face; with the ability to be adaptable, empathetic, solution focused, non-judgemental and advocate for social justice outcomes.
  • Experience in all aspects of case management, case plans and evaluations with a clear understanding of strengths based, client centred and trauma informed practice.
  • Highly skilled in communication, mediation and conflict resolution skills.
  • Ability to respond and act calmly and assist distressed young people sensitively, confidently and with dignity and respect.
  • Demonstrated competence in networking, community liaison and community capacity building; working with a range of appropriate professionals in youth related services.
  • Demonstrated understanding of child protection issues with capacity to identify levels of risk requiring statutory child protection intervention.
  • High level of written and computer literacy, demonstrated knowledge of and experience using data collection systems, tools and data evaluation processes, including the ability to complete comprehensive reports.
  • Capacity to work with minimal supervision with a high ability to meet deadlines, manage competing priorities and tasks.
  • Demonstrated ability to work within professional boundaries, integrity and ethical standards.
  • Must be able to be On-Call (after business hours) for a 7 days period, bi-monthly, on a rotating roster.
  • Possess a valid NSW Working with Children Check and Australian Federal Police Check.
  • Possess a current NSW driver licence.
