About the Role Fulfil the accountabilities and responsibilities of this role in accordance with Metro South's purpose and objectives, as outlined below. This position, collaborating with members of a multidisciplinary team, will: Support and liaise with hospital and community Chronic Disease services to coordinate health care service delivery and information for Aboriginal and Torres Strait Islander people and their families. Link Aboriginal and Torres Strait Islander people and their families to appropriate local health and social service providers.
