Overview
You will provide a holistic support service to Aboriginal clients in the Gawler community. Working in a collaborative and participative framework to increase continuity of care across the health setting, you will strive to address social issues and promote better outcomes for Aboriginal people through the facilitation of Well Health Checks. You will play a vital role in networking and maintaining collaborative partnerships between service providers, non-government agencies and the community throughout the Gawler service area. Contributing to service development activities and evaluating service effectiveness, you will assist with monitoring care plans to ensure appropriate and relevant care outcomes are achieved on a daily basis. SA Health is committed to improving the health of Aboriginal South Australians. Promoting positive health in the community, we are developing culturally appropriate models of care for Aboriginal Health and extending Aboriginal community engagement. Increasing access and equity to health services, we are committed to addressing the social determinants of Aboriginal health.
