Dementia and Aged Care Services (DACS) is a new provider of Community Care Services in NSW. The purpose of the Intake Support Coordinator role is the intake of clients from the My Aged Care portal, initial contact with clients, screening and assessment. All functions of this position occur in accordance with the organisations mission, objectives, and priority outcomes, ensuring and maintaining the integrity of the organisation's reputation and core values.
Responsibilities include:
- Phone based role being the first point of contact for current and potential clients
- Assist with the assessment and onboarding of new clients as well as undertaking needs assessments and assist with basic reviews of existing clients
- Efficiently process aged care and allied health client intakes and referrals in a timely and accurate manner
- Liaise with the appropriate Manager or Coordinators to support prompt onboarding of clients and completion of all required documentation as requested.
- Ensure all queries from the client and/or their family are answered in a timely manner
- Completion of client in-take on-boarding documentation and administration as a result of initial sign ups and assessments
- Build relations with key stakeholders, Regional Assessment Services, My Aged Care (MAC) and other service providers to better service provision to clients
About You
- Demonstrated experience and knowledge of working in a client intake (ideal) or customer/client services
- Must be tech savvy, can confidently navigate databases
- Ability to network effectively and collaborate with a wide range of professionals including referral agencies for all funding types.
- Work independently and be self motivated as well an effective member of the support services team
- Work empathetically with clients to ensure the suite of services are designed to meet their ongoing needs
Qualifications
- Relevant qualification in a health care field or transferrable skills and/or experience highly regarded
- Current National Police Certificate clearance and willingness to renew it every 3 years as part of your ongoing employment;
