About the role

The ACP clinican will provide work with clients, families, carers, GPs, Nurses and other clinicians across the Outer East catchment to support the implementation of Advanced Care Planning. 

About you 

To be successful in this role you will possess:

Essential:

• Bachelor of Nursing or other recognised equivalent formal qualification leading to condition free registration in Australia
• Registration with the Nursing and Midwifery Board of Australian (NMBA) via AHPRA
• Understanding of the principles of advance Care Planning
• Experience in providing patient care in situations where advance Care Planning is applicable
• In-depth knowledge of the Medical Treatment Planning & Decisions Act 2016 (Vic)
• Victorian Driver’s License
• An understanding of the Voluntary Assisted Dying legislation

Desirable:

• Post Registration or Post Graduate qualification or further study in a relevant clinical or academic field or working towards
• Research publications and presentations at conferences
