Position SummaryAdvanced Care Planning Nurse

• Perm Part Time - 32 hours fortnightly - $54.64 hr
• Work in the Outer East and Yarra Valley Areas
• Community Health Role based in Healesville

We provide a range of emergency, surgical, medical and general healthcare services, including maternity, palliative care, mental health, drug and alcohol, residential care, COMMUNITY HEALTH and state-wide specialist services to people and communities that are diverse in culture, age, socio-economic status, population and healthcare needs.About the roleThe ACP clinician will provide work with clients, families, carers, GPs, Nurses and other clinicians across the Outer East catchment to support the implementation of Advanced Care Planning.About youTo be successful in this role you will possess:Essential:

• Bachelor of Nursing or other recognised equivalent formal qualification leading to condition free registration in Australia
• Registration with the Nursing and Midwifery Board of Australian (NMBA) via AHPRA
• Understanding of the principles of advance Care Planning
• Experience in providing patient care in situations where advance Care Planning is applicable
• In-depth knowledge of the Medical Treatment Planning & Decisions Act 2016 (Vic)
• Victorian Driver’s License
• An understanding of the Voluntary Assisted Dying legislation

Desirable:

• Post Registration or Post Graduate qualification or further study in a relevant clinical or academic field or working towards
• Research publications and presentations at conferences
