SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.The Aftercare Peer Navigator will draw upon their lived experience of mental health concerns and their personal recovery journey to support personal recovery, service knowledge and service navigation for South Eastern Sydney Local Health District (SESLHD) mental health consumers. The goal of the peer worker component of the role is to support the consumer through their mental Health Service journey by providing individual consultations aimed at facilitating more timely and purposeful movement through the mental Health Service utilising choice, and self-determination.The position will work in conjunction with the Emergency Department (ED), Psychiatric Emergency Care Centre (PECC) and Mental Health Service Staff.The Aftercare Peer Navigator role will support the integration of aftercare services including The Way Back Support Service in SESLHD. The Way Back Support Service, delivered by NEAMI National, provides community-based assertive psychosocial support for people following a suicide attempt or suicidal crisis who have been referred from an acute care or community mental health setting.This role will collaborate with relevant hospital and community mental health and suicide prevention services to ensure access to aftercare support is available for people who need it.Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.All workers and new recruits are required to receive 2 doses of a Therapeutic Goods Administration approved or recognised COVID-19 vaccine to commence employment/ engagement or continue to work within a NSW Health service.A worker and new recruit will be considered compliant if they have a medical contraindication to all available Therapeutic Goods Administration approved or recognised COVID-19 vaccines and provide medical contraindication evidence in line with the policy requirements.In addition, all Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza or COVID-19 vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.SESLHD is committed to creating a workplace that reflects the diversity of our community.   • Demonstrated knowledge and appreciation of contemporary approaches to suicide prevention and trauma informed service delivery models.
  • Demonstrated ability to work independently or as part of a team in performing a range of management and administrative tasks within allocated timeframe.
  • Personal lived experience of mental health challenges and/or suicidal crisis, and demonstrated ability to meaningfully and purposefully use lived experience to provide peer support to people at risk of suicide.
  • Demonstrated excellent written and verbal communication skills with the ability to prepare reports, collect data and co-facilitate education.
  • Demonstrated effective interpersonal and communication skills to collaborate with key stakeholders.
  • Demonstrated knowledge of primary, secondary and tertiary mental Health Services, and ability to utilise appropriate support such as crisis response, risk formulation and safety planning.
  • Demonstrated ability to be flexible and adapt work practices to meet current and emerging business needs.
