Our Preventative Integrated Care Service team includes senior clinicians across multiple disciplines and specialties (endocrinology, cardiology, respiratory), who collaborate to provide short-term holistic care to patients at high risk of hospitalisation

• Permanent Part-time opportunity (0.5 FTE)

About the role

The Preventative Integrated Care Service delivers (PICS) will provide a rapid response (review within 24-48 hours) for intensive sub-acute medical management, delivered by a specialised medical, nurse practitioner and allied health team with a disease specific focus through flexible and varied modalities. As the Occupational Therapist Senior, the role will involve providing high level assessment and intervention to patients receiving care from PICS. As a senior member of PICS this role will also provide clinical leadership and service development.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Clinical Expertise: A high level of knowledge, expertise, and skill in contemporary clinical practice standards for adult physical occupational therapy services relevant to PICS as recognised by clinical experience, professional development activities, post graduate education or formal qualifications.
  • Team Focus: Broad understanding of the continuum of care, and well developed interpersonal, communication and negotiation skills, including the ability to work autonomously and flexibly with initiative in a multi-disciplinary team environment.   
  • Patient Focus: Build rapport with patients and demonstrate empathy, patience and respect in delivering a flexible service that meets patient's needs, in accordance with organisational goals.
  • Quality Improvement: Applies clinical evidence that support continuous improvement, including the ability to initiate, plan and evaluate local quality and service improvement activities.
  • Developing Self and Others: Demonstrated commitment to education and training including supervision of students and staff, as well as ongoing continuing professional development.
  • Leadership and Organisational Skills: High level organisational skills, including the ability to effectively prioritise clinical caseloads and non-clinical tasks, and the ability to provide leadership and direction to students, HP3 clinical staff and assistants.
