Aboriginal Community Services (ACS) are a non-profit provider of residential Aged Care and home and community services. We provide Aged Care services to our Elders / Clients across South Australia in Metro, rural and remote locations.
About the role
The main objective of this role is to provide service coordination and support for the client to deliver their service plan, ensuring a client driven service delivery model that places client choice and cultural preferences at the centre of service delivery. You will work with a close knit team of professionals to manage client services in accordance with the CHSP and HCP guidelines and legislation to promote and achieve positive client outcomes. This job is not available to work from home.
Responsibilities:
You will be responsible for the provision of Case Management for complex needs, the intake of new clients and developing, implementing and reviewing client service plans and budgets along with managing individualised funding under the home care package program (HCP) and Commonwealth home support program (CHSP). The understanding of chronic health and co-mobilities plays an important part of this role. You will also regularly evaluate client care and support and monitor changing or emerging needs. ACS thrives on continuous improvement opportunities therefore you will take active part in team meetings to discuss continuous quality improvement activities. You will also support the Community Support Workers and agency staff to safely deliver culturally appropriate care and support.
About you:
To be successful you will possess excellent communication and organisational skills. You have the knowledge and understanding of service and assessment models that support older people to live independently in the community. You will also have a natural ability to actively engage clients in meaningful goal setting and care planning activities. Your strong multitasking skillset, attention to detail and accuracy and ability to work in a fast-paced environment will see you succeed in this role. Previous experience working with Aboriginal people be highly regarded but not essential.
You have a current national police check or willing to obtain one. You will also possess a current Driver’s License.
