About the role of Aged Care Advisor
- Full Time 38 hours per week
- Monday to Friday
- Role based in Mile End right on the bus stop
- Some car parking available
- Salary SACS L4.1 $82045 per annum plus superannuation and paid leave

The main objective of this role is to provide service coordination and support for clients on CHSP and HCP/Home Care Packages ensuring a service delivery model that places client choice and cultural preferences at the centre of service delivery.

Responsibilities:
You will be responsible for the provision of Case Management for complex needs, the intake of new clients and developing, implementing and reviewing client service plans and budgets along with managing individualised funding under the Home Care package program (HCP) and Commonwealth home support program (CHSP). The understanding of chronic health and co-mobilities plays an important part of this role. You will also regularly evaluate client care and support and monitor changing or emerging needs. ACS thrives on continuous improvement opportunities therefore you will take active part in team meetings to discuss continuous quality improvement activities. You will also support the Community Support Workers and agency staff to safely deliver culturally appropriate care and support.
The ideal candidate:
Experience in a Home Care Coordinator or Senior Home Care Coordinator/Aged Care Advisor role, and working with CHSP and HCP clients
Previous experience working with Aboriginal people
National police check
