Overview

The Aged Care Assessment Team (ACAT) Coordinator will deliver nursing and/or midwifery services across diverse healthcare settings, leveraging expertise honed through experience and/or advanced studies. Progress from proficient to adept practitioners, assuming responsibility for upholding practice standards, overseeing delegated tasks, and mentoring less seasoned colleagues.

Support the Nursing/Midwifery Nurse/Midwife Unit Manager or equivalent in guiding and leading nursing/midwifery teams within the ward/unit/service. Assigned a portfolio, incumbents will:

  • Foster seamless care continuity and coherence in collaboration with fellow AN/MUM colleagues and the Nurse/Midwife Unit Manager or equivalent.
  • Aid in implementing practice enhancements under the guidance of the Nurse/Midwife Unit Manager or equivalent.
  • Assist in managing ward/unit/service operations, encompassing performance evaluations, recruitment, staffing logistics, leave administration, scheduling, task delegation, and attendance oversight. Additionally, contribute to financial planning and supply management.

Take charge of developing, coordinating, executing, and evaluating the LCLHN Aged Care Assessment program spanning acute, residential, and community settings. Provide specialized expertise in Aged Care Assessment, enriching the team's capabilities in this domain.

Applicant Information

Must be registered or eligible for registration as a Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.

Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.

  • National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. Aged Care employment or work involving vulnerable clients)
