Overview
The Assessment CoordinatorAustralian National Aged Care Classification (ANACC) will provide support to the Aged Care Facilities of Barossa Hills Fleurieu Local Health Network Inc (BHFLHN) regarding the ongoing ANACC assessment of residents. A key focus of the work involves supporting the continuation of the ANACC assessment initiatives that lead to improved clinical and business systems in Residential Aged Care Facilities.
The position will contribute to the planning, development, and monitoring of ANACC assessments across all Aged Care sites of BHFLHN and Contribute to a range of strategic Aged Care initiatives with regards to Commonwealth reforms and system change.
The position will develop key relationships with the Aged Care leadership team and staff of RACFs to continue monitoring the completion of ANACC assessments. The role will contribute to staff education at all Aged Care sites to improve the awareness of staff in regard to care planning for the residents.
