The successful applicant will be working with a dedicated team of ACAT Assessors as well as the TACP team, Dementia Support, Psychogeriatric Service, Podiatrists, ASET team, Occupational Therapy, Speech Pathology & Physiotherapy Service, Discharge Planner & AARCS Nurse. These positions are supported by the administration team and Health Service Manager.

About the role:
This position requires the ability to conduct ACAT Assessments in the community or hospitals across the Lower Mid North Coast Sector, evaluating the care needs of individuals and determining access to government subsidised Aged Care services. You will be required to conduct assessments using a mobile computing environment.
The successful applicant will be supported with the appropriate training to be an ACAT Assessor, if required, as well as additional support once this is completed, to fulfil the role. You will be required to complete the Integrated Assessment Tool (IAT) training for ACAT Assessors and have a passion for continuing professional development.

Requirements:
- Must hold a qualification in one of the following: Occupational Therapy, Physiotherapy, Nursing, Social Work, which provides eligibility or membership of the relevant professional association / provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA).
- Unrestricted NSW Drivers Licence and willingness to travel across the lower mid north coast as required for community assessments.
