Follow the link to find out what its like to live, work and play in our wonderful region:About the role:Community Interlink is a partnership between regional hospitals and health services from central and northern Victoria and southern New South Wales.The alliance assures the availability of locally-governed Home Care Packages and NDIS supports to communities within our region.Community Interlink has an opportunity for a Aged Care Case Manager NDIS Support Coordinator to Join the Seymour or Shepparton Team. This supportive team will nurture you and help you to upskill to build your future in this industry. The role provides guidance and support to our vulnerable community members to identify and support independence and support attainment of goals.This role will support either Shepparton or Seymour teams.We are looking for:
- An experienced Aged Care Case Manager or NDIS Support Coordinator
