This supportive team will nurture you and help you to upskill to build your future in this industry. The role provides guidance and support to our vulnerable community members to identify and support independence and support attainment of goals.This role will support either Shepparton or Seymour teams.We are looking for:

• An experienced Aged Care Case Manager or NDIS Support Coordinator
