We currently have an exciting opportunity for what we call an Aged Care Clinical Mentor to join the Clinical Governance team on a permanent full time basis.
This role will support our three Aged Care homes in the South West - Elanora in Bunbury, Fields in Eaton and our new Aged Care home in Dalyellup.
You will be part of an encouraging and supportive team that has one aim, that is improving the clinical care provided to our residents!
About the Role:
Our Aged Care Clinical Mentors work collaboratively with clinical staff to initiate the Aged Care Clinical Mentor Model of Change to enable positive outcomes for staff and residents across Aged Care services by embedding clinical best practices.
Responsibilities include:
- Providing clinical leadership, direction and mentoring to staff
- Assisting operational management to identify clinical areas of priority through auditing and review of clinical trends and feedback
- Supporting staff to implement and maintain changes in clinical practice with assistance of key stakeholders including facility management, learning & development and information technology teams
- Active involvement in the identification and implementation of continuous improvement initiatives utilising mentoring in nominated clinical priority areas
- Maintaining a safe environment
You're passionate about working collaboratively with other health professionals to improve care outcomes and bring excellent clinical assessment, diagnosis and treatment planning skills. In addition to a current, unrestricted AHPRA registration you will have:
- Minimum of 3 years experience in an Aged Care environment
- Working knowledge of the Aged Care Standards
- Demonstrated ability to work independently
- Excellent interpersonal skills and communication, both written and verbal
- A sound understanding and commitment to the person-centred approach in Aged Care
- Interest in developing others and pursuing own development
- Ability to learn from experiences and share them
- Good understanding of evidence-based practice and critical reasoning
