Join Us
We are looking for compassionate, enthusiastic people to join our growing team. We have opportunities in Home Care and Residential Care as an Aged Care Coordinator (Consultant). You will work with clients providing them with information and guidance that will assist them to navigate the Aged Care system, identify appropriate home care or residential Aged Care facilities and assist the family to identify the best Aged Care services available to meet their needs.
Your weeks will be varied as you liaise with clients, social workers, hospital staff, Aged Care facilities and other key people involved in the decisions around future care needs of the client. Your role will involve assisting clients with the completion of paperwork, researching and reviewing facilities, arranging facility tours and supporting the transition to residential care.
You will also be required to meet and introduce My Care Path to local ACAT and Hospitals, Financial Planners and other referral sources.
Experience for the role
You will be dealing with a diverse range of people so empathy, strong interpersonal and communication skills are essential. You will need to be self-motivated and able to work independently towards an outcome.
We use a database so intermediate computer skills are needed.
Ideally, you will have experience in the Aged Care sector. Your experience may have come from being a Placement Consultant, Admissions Coordinator, Registered Nurse, Home Care Case Manager or similar. This role may also suit Registered Allied Health Professional or those working toward registration.
Must Haves
• Current Australian Drivers Licence
• Own vehicle with comprehensive insurance - you will be reimbursed for use.
• National Police Check will be required prior to commencement
