We have an opportunity for an Aged Care Navigation Specialist to join our Central Services team. Reporting to the Aged Care Navigation Team Manager, this position is based at our Pier Street office. You will be required to be available Monday to Friday, 9:00am – 5:00pm (start and finish times can be flexible). This role is offered on a permanent, full time basis.

About the role

The Aged Care Navigation Specialist (ACNS) will be empowered to provide information, support and advocacy to all customers as they navigate the complex Aged Care system. The ACNS works with customers to understand their needs, identify available options, and make informed choices about their care. Duties will include:

  • Provide customers the information and assistance they require about the Aged Care system, including eligibility requirements, services and supports and accessing My Aged Care Service and Support Portal to support informed decision-making regarding the delivery of their care and services across the continuum of Aged Care
  • Initial focus for this role will be to manage all enquiries for Home Care services and will be based alongside and work closely with our Home Care team. Over time the role will transition to manage all enquiries coming into Bethanie – Home Care, Retirement Village and Residential Aged Care.
  • Manage waitlists and vacancy boards in Epicor (CRM system), including working closely with Home Care, Village and Facility Managers, including looking at Home Care as an interim option for customers in the waitlist
  • Engage and develop strong relationships with internal teams from admissions coordinators, case managers and other healthcare professionals, to facilitate and optimise the client/resident on-boarding process
  • Monitor and communicate changes in legislation that affect subsidies and/or funding
  • Review the NSAF and / or Integrated Assessment Tool, including identifying the prospective AN-ACC classification, and highlighting any concerns in the delivery of care and services either in the customer’s home or in one of our Villages or Facilities.

About you:

You are a highly experienced customer service professional and bring a strong understanding of the Aged Care industry. You can relate to a diverse range of stakeholders and have:

  • Ability to develop relationships with our clients/residents, their families/ representatives, and staff
  • Excellent customer service skills, a passion for the wants, desires and need of older Australians
  • Excellent verbal and written communication skills
  • Strong understanding of MS Office suite of products as well as database/CRM software
  • Ability to work independently and autonomously, but also as part of a team
  • Knowledge of Home Care funding programs, Medicare claim, AN-ACC classification and the My Aged Care Service and Support Portal is desirable
  • Willingness to uphold the Mission, Vision, and Values of The Bethanie Group Inc
  • Current National Police Clearance, or willingness to apply for one
  • Willing to obtain required vaccinations – COVID 19 (minimum of 3 doses) and annual flu vaccination
