We are looking for a dedicated and skilled Social Worker to join our busy and rewarding team!
About the Role:
The Social Work clinician participates as a member of the GEM at Home team, which provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission. This includes the provision of geriatrician-led, multidisciplinary assessment, restorative interventions, and sustainable care planning in collaboration with the older person, their family, carer and existing community services involved in their care.
This position encompasses both the GEM@Home hospital substitution program and Community GEM case management streams of the service and is based at the Repat Health Precinct.
As the Social Worker, your duties may include:
- Short Term Case management.
- Independently assessing clients’ abilities, problems and needs using a variety of interdisciplinary assessment tools.
- Planning and implementing evidence-based intervention programs.
- Implementing outcome measures appropriate to the client population to evaluate efficacy of therapy programmes.
- Arranging and supporting Geriatrician assessments.
- Initiation, support and navigation of the SACAT process from application to outcome.
- Prescribing, arranging and linking with appropriate care providers.
- Communicating with clients’ family, carers, and other service providers.
About you:
- Appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Demonstrated experience in clinical management within geriatric services, including program evaluation and service development, combined with a broad base of experience in social work practice.
- Demonstrated high level competency and extensive knowledge in the application of a wide range of social work assessment and therapeutic interventions as a member of an interdisciplinary team to clients with complex care needs.
- Demonstrated skills and emerging abilities in leadership; supervision of staff, flexibility, working independently and within an inter-disciplinary team, managing variable workloads and competing priorities and adapting to changing service provision needs.
