About the role:
As an AOD Youth Outreach Worker, you will operate within a harm minimisation framework, providing clinical assessment and case management to young people in their own environments or at BCH facilities.
This role involves working with both mandated and voluntary clients, participating in health promotion and community education sessions, and ensuring the continuity of care through assertive outreach and effective service linkages.
Key responsibilities:
- Provide assertive outreach to engage clients and facilitate linkages to other services.
- Offer ongoing support and case management to young people in an accessible ‘open door’ capacity.
- Develop and maintain effective partnerships with internal and external service providers.
- Deliver high-quality engagement, assessment, treatment, and support services using best practice principles.
- Participate in health promotion activities and provide education and information sessions.
What you will bring to the team:
You are a dedicated professional with a strong commitment to harm minimisation and Youth Outreach. You have excellent communication and interpersonal skills, and are adept at working with diverse populations. You are organised, self-motivated, and able to work both independently and as part of a team. Your experience in AOD and/or similar services, combined with your knowledge of best practice principles and evidence based treatment, makes you an ideal candidate for this role.
