Be a part of a multidisciplinary team of professionals (Nurses, Social workers, Psychologists, Speech pathologists, Occupational Therapists, and Physiotherapists) who are passionate and committed to the development and delivery of programs and Services for children, young people, and families in the community. As the IHSHY Outreach Worker, you would be responsible for providing comprehensive health assessment, generalist counselling, care planning and coordination, health education, and advocacy support to young people (aged 12-25 years) who are experiencing or are at risk of homelessness.

Responsibilities:
As the Innovative Health Services for Homeless Youth (IHSHY) Outreach Worker your responsibilities will include:
  • Providing assertive outreach health and wellbeing support to young people in a range of community settings, including private homes, public housing, transitional/crisis housing, youth and community services, schools, and a range of other health care settings.
  • Advocating for young people, by identifying and addressing barriers that prevent access to social and primary health care services.
  • Collaborating with internal and external service providers to ensure that young people have coordinated access to the services and supports they need.
  • Providing secondary consultation, and seeks opportunities for partnership with local youth services, councils, schools, and other key stake holders and service providers.
  • Participating in the planning, development, implementation, and evaluation of health promotion group programs, services, and materials offered to young people in the community
  • Requirements and attributes:
To be successful in this role you will have:
  • A tertiary qualification in nursing or social work.
  • Excellent knowledge in adolescent health and development, and the impacts of social isolation and homelessness.
  • Demonstrated experience working with marginalised young people with complex social and primary health care needs.
  • The ability to work both independently, and collaboratively, as part of a multidisciplinary team.
  • A clear understanding of the principles of community health and assertive outreach models of service delivery.
  • Experience and/or a commitment to working with Indigenous and CALD families.
