Responsibilities:
As the Innovative Health Services for Homeless Youth (IHSHY) Outreach Worker your responsibilities will include:
- Providing assertive outreach health and wellbeing support to young people in a range of community settings, including private homes, public housing, transitional/crisis housing, youth and community services, schools, and a range of other health care settings.
- Advocating for young people, by identifying and addressing barriers that prevent access to social and primary health care services.
- Collaborating with internal and external service providers to ensure that young people have coordinated access to the services and supports they need.
- Providing secondary consultation, and seeks opportunities for partnership with local youth services, councils, schools, and other key stake holders and service providers.
- Participating in the planning, development, implementation, and evaluation of health promotion group programs, services, and materials offered to young people in the community
- Requirements and attributes:
- A tertiary qualification in nursing or social work.
- Excellent knowledge in adolescent health and development, and the impacts of social isolation and homelessness.
- Demonstrated experience working with marginalised young people with complex social and primary health care needs.
- The ability to work both independently, and collaboratively, as part of a multidisciplinary team.
- A clear understanding of the principles of community health and assertive outreach models of service delivery.
- Experience and/or a commitment to working with Indigenous and CALD families.
