Join the community based Transition Care Program (TCP) multidisciplinary team in this unique role.
The TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. Providing quality, evidence-based care, the program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. Community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and to finalise and access their longer term care arrangements or maintenance at home. The TCP can be provided as a bed based or as a home based service. By offering case management, low intensity therapy and personal support, TCP allows older people more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity and finalise their long term care arrangements.

A Day in the Life
Our team of highly experienced Health Professionals work to provide timely, person-centred interventions to prevent deterioration of health, and admission to hospital and or residential care. This is a unique opportunity to join our large, friendly team, with a highly varied caseload (where no two days will be the same!). You will be part of the wider Monash Health Allied Health Assistants network with access to all benefits including in- house professional development, supervision, and broad range of clinical services.

What you need:
Certificate II Allied Health Assistance
Ideally greater than 2 years AHA Experience (desirable)
Current Driver’s License
