About the role
As part of the Community Health, Rehabilitation and Palliative Care directorate this role will deliver services to the multi-disciplinary Community Health Primary Care services. The position is responsible for the continuous improvement and delivery of high quality services based on the qualifications of the successful applicant/s. To be successful in this role you must work in Occupational Therapy, Physiotherapy, Speech Pathology, social work, Dietetics and/or Podiatry. All activities of the position will reflect current best practice standards and support consumers and cares to achieve an optimum level of health independence through:
- Comprehensive, coordinated and integrated multidisciplinary services
- Provision of consumer-centred care to community members from a broad range of backgrounds
- Contribute to meeting the program and allied health key performance indicators (KPIs)
