Barwon Health provides consumer-centred multidisciplinary care across a number of Community Health Centre sites including Belmont CHC, Newcomb CHC, Surf Coast CHC and Corio CHC. These roles will be located at a primary Community Health Centre location however may be moved to other Community Health Centre sites depending on service delivery needs.

About the role

As part of the Community Health, Rehabilitation and Palliative Care directorate this role will deliver services to the multi-disciplinary Community Health Primary Care services. The position is responsible for the continuous improvement and delivery of high quality services based on the qualifications of the successful applicant/s. To be successful in this role you must work in Occupational Therapy, Physiotherapy, Speech Pathology, social work, Dietetics and/or Podiatry. All activities of the position will reflect current best practice standards and support consumers and cares to achieve an optimum level of health independence through:

• Comprehensive, coordinated and integrated multidisciplinary services
• Provision of consumer-centred care to community members from a broad range of backgrounds
• Contribute to meeting the program and allied health key performance indicators (KPIs)
