CAAPS is an Aboriginal Community Controlled organisation delivering a range of important community services to people through activities and services that reflect our CORE values of caring, openness, respect and empowerment. You will be part of an exciting program delivering a new approach to providing AOD and Youth Services.
About the position
Provide support and trauma-sensitive care planning and goal setting aimed at the empowerment of adults who have suffered the impact of substance use of self or family members.
Some of your responsibilities will be:
- Develop relationships with clients and their families to achieve therapeutic and lifestyle goals, whilst maintaining professional boundaries.
- Provide high quality support to focus on recovery using a relational, trauma informed approach aimed at long-term sustainable outcomes, including follow up and aftercare.
- Conduct client assessments and screens as required under the guidance of the AOD Social Worker.
- Always maintain an awareness of client’s needs and wellbeing, including participation in internal handover procedures, assisting with medication and provision of transport.
- Provide on call support outside of business hours on a rostered basis, in line with CAAPS Policy & Procedures.
- Facilitate case planning in consultation with all parties and in line with CAAPS current practice models and adhere to legislative requirements of role.
- Provide culturally appropriate services as relevant to the position.
- Display a strength based, client centred attitude with consideration to CAAPS values.
- In the course of other duties, support the establishment of networks and relationships with remote and urban-based communities and organisations.
- Client case management files including case management plans, reviews, and discharge plans, including relapse prevention.
Your skills, experience and qualifications include:
- Experience in effectively delivering community services to Aboriginal and/or Torres Strait Islander people.
- Demonstrated ability to modify communication styles and techniques to meet client’s needs (Multilingual ability an advantage)
- Demonstrated ability to work positively both as part of a multidisciplinary team and with individuals.
- Demonstrated written communication, competent administrative and documentation skills.
- Demonstrated computer skills including MS Suite, email, data collection/input.
- Dedicated and careful; high level of accuracy and attention to detail
- Sound knowledge of the issues impacting Aboriginal and Torres Strait Islander people
- Demonstrated understanding of current issues impacting the delivery of AOD treatment for individuals and families affected by substance misuse.
- A sound knowledge of current local and national strategies related to the AOD field.
- Ability to work with minimal direct supervision.
- Willingness to maintain a drug free lifestyle.
- Current First Aid Certificate, or willingness to obtain
- Current NT Driver’s License (minimum C Class)
- Current Criminal History Check
- Current Ochre Card (Category E)
