About the role
The ACAT assessor stands within the nursing department of the CCH/ACAT multidisciplinary team. The primary purpose of the position is to provide holistic, comprehensive assessment, support and management to the target population in the St George Geographical area in the community, hospitals and aged care facilities as applicable.
Key Responsibilities
ACAT assessments completed within the nominated KPI
100% accurate documentation in the My Aged Care system and the electronic medical record
Maintaining CPD points
Completion of Mandatory Training
What you bring
Registered Nurse with current registration with the Australian Health Practitioners Regulation Agency (AHPRA), plus CNS 1 criteria found at www.health.nsw.gov.au/careers/conditions/Awards/nurses.pdf as well as a current unrestricted Class C drivers licence
Minimum 3 years' post graduate experience working with older people in an acute, aged care and /or community setting.
Knowledge of Aged Care Systems as well as Aged Care issues and experience working with clients from diverse cultural backgrounds.
Demonstrated ability to provide education to clients, peers and carers as well as commitment to ongoing self-education and best practice.
Demonstrated high level of interpersonal, written and verbal communication skills including computer literacy.
Demonstrated ability to work autonomously and as part of a multidisciplinary team.
Demonstrated knowledge of Quality Improvement initiatives and Work Health and Safety principles relative to community service delivery.
Ability to work within the Mission and Values of Calvary Health Care.
