Associate Nurse /Midwife Unit Manager Clinical Support / Clinical Support Officer

• Barossa Hills Fleurieu Local Health Network - Country Referral Unit, Nuriootpa
• Salary: RN/M2A: $100,343 - $107,279 p.a. (Pro-rata) / AHP2: $86,950 - $100,729 p.a. (Pro-rata)
• Temporary Part Time position available, working up to 31 January 2025
• If employed at RN2 classification, working 24 hrs p/wk & if employed at AHP classification working, 22.5 hrs p/wk

Please note that this position is multi-classified as either an AHP2 or RN/M2A.

Overview

Clinical Support Officer is accountable to the Program Manager, Country Referral Unit (CRU), Country Health SA for active participation in the Country Referral Unit Project, throughout Regional South Australia, on behalf of the 6 Local Health Networks – Barossa Hills Fleurieu (BHFLHN), Eyre and Far North (EFNLHN), Flinders and Upper North (FUNLHN), Limestone Coast (LCLHN), Riverland Mallee Coorong (RMCLHN), Yorke and Northern (YNLHN). CRU operates over a 7 days 8am to 6pm.

 The main components to the role are:

• Providing clinical knowledge and expertise to assist Intake Officers in their intake role
• Effectively manage the referrals, related documentation and data of the CHSA Transition Care (TCP) and Country Home Link (CHL), Community and Home Services Program (CHSP), My Aged Care Portal  in line with SA Health and DHW guidelines
• Supporting consistent provision of quality TCP, CHL, CHSP, MAC and allied health referral and allocation processes within the Country Referral Unit Project
• Enhancement of skills and knowledge of Intake Officers, in providing needs assessments.
• Strong understanding of the Community and Home Services Program (CHSP) Sector, My Aged Care Portal, Aged Care Assessment Program, and Community Services Sector.
