Job description
Western Health is seeking an experienced Registered Nurse to join the dynamic Central Access Unit (CAU) team on a permanent part-time basis, to work in the care coordination role. The CAU Care Coordinator, improves access to Western Health at Home services by managing intake and referral processing for patients at a single point of entry. We are looking for a person who is passionate about integrated care and patient flow to triage, direct, reject or redirect referrals to community programs. The Care Coordinator, when working in the post discharge area, monitors clients by telephone and follows up on any clinical issues should the need arise, for patients post discharge from hospital. The role ensures that patient care is being delivered in accordance with the arranged care plan and provides a single point of contact for the consumer. The role is permanent part-time and will be required to work across a 7-day rotating roster including public holidays.
To be Successful in this Role you have:
a minimum of 5 years post graduate acute and community experience
highly developed problem solving and negotiation skills
well-developed interpersonal and communication skills
ability to work in a faced paced environment in a flexible way
a comprehensive knowledge of community care options available in the western suburbs
experience in discharge planning and/or clinical triage would be highly regarded
