About the Role:
Intereach is seeking a dedicated individual with highly developed communication skills to join the Aged Care Services team in the role of Care Coordinator HCP.
This position will be responsible for coordinating and facilitating in home services and supports to older people who are eligible to receive Home Care Packages. Care Coordinators will work as part of a team to promote independence and community participation for older people to help them remain in their own homes.
The successful applicant will have well-developed computer skills and demonstrated experience with the Microsoft Office Suite, excellent time management and organisational skills, an understanding of the issues facing older people and their carers and a passion for making a difference in the community. You will have a strong commitment to upholding the Intereach values and a great knowledge of services available in your community. If this sounds like you, we would love to hear more about you.
The position is full time - 38hrs per week and can be located in Albury or Corowa
This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with an annual starting salary of $82,043.52 + Superannuation + Salary Packaging.
