Job description

About the Organisation

Enhance the lives of older Australians by joining the Life Without Barriers team. Guided by every client’s individual needs and interests, our personalised in-home Aged Care services focus on empowering every person to maintain their home comforts, health, and connections within their community.

Life Without Barriers is a leading social purpose organisation of 8,000 employees working in more than 500 communities across Australia. We support children, young people and families, people with disability, older people and people with mental illness. We work with people who are homeless and refugees and asylum seekers. Join a community of people dedicated to breaking down barriers.

We want to employ people who reflect the diversity of our clients to ensure we can support each client's individual needs and wants. We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.

About the Role

As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services across Adelaide and surrounding local areas. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families. Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.

You will provide a high level of service to ensure our clients and their families feel cared for and supported. This is an exciting opportunity to join a supportive team that are passionate about the work they do and enjoy working collaboratively. This is a Permanent Full Time Hybrid opportunity located in Unley, where you will be able to work some days each week from home.

Key Responsibilities

  • Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
  • Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice Leads
  • Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
  • Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community

Skills & Experience

  • Demonstrated case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholders
  • High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systems
  • High level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.
  • Ability to multi-task and work autonomously in a fast paced environment
  • Previous experience in Community Services will be highly regarded
  • A current unrestricted Australian Driver Licence

Benefits

  • Join one of Australia’s leading social purpose organisations
  • Career pathways across a national organisation
  • Attractive remuneration package with a monthly rostered day off (RDO)
  • Hybrid opportunity, work some days each week from your home
  • Up to $15,900 of your annual salary tax free through Salary Packaging
