About the role:
We currently have multiple opportunities across South Australia for passionate Social Workers to work within our Australia-wide team. The successful candidates will fit the criteria by working alongside the leadership team, helping develop and implement clinical services, educate and case manage veteran (DVA) patients.
We're looking for Social Workers who genuinely care and are eager to collaborate as a team to expand our clinics and serve our community. We value individuals who demonstrate strong initiative, honesty, and a thorough clinical approach, while also being capable of managing a diverse caseload.
Responsibilities:
- Provide education, information, health coordination and support to veterans.
- Conduct thorough assessments of patient’s – medical history, current health conditions and treatment goals.
- Building and maintaining rapport with patients.
- Facilitate community development initiatives, mobilizing resources and
organizing programs to address social issues, promote social justice, and
build stronger communities. - Provide generalist counselling for social and emotional support.
- Assist veterans in completing all required documentation, forms and
applications needed for enrolment in any of the Pro-Active Health services
offered. - Focus on assisting people day to day with effective communication and
service.
Requirements for the role:
- Possess a relevant Social Work degree
- Maintain current registration with AASW (Australian Association of Social
Workers) - Demonstrate a genuine interest in veteran’s welfare
- Showcase proven high-standard written and verbal communication skills
- Proficient computer skills and able to work with various computer software programs
