About the Role

We are seeking an experienced Care Coordinator (Registered Nurse) to support development and implementation of our new Better Together at Home program, providing services under My Aged Care – Home Care Packages. Ideally, you will have a strong mix of case management and nursing skills and enjoy working with aged clients in the community.

Your will utilise your nursing and Care Coordinator experience and will include the provision of aged care nursing procedures, training and supporting field staff, managing client intake, assessments and care planning for Better Together at Home clients, case management, managing client budgets, as well as coordinating and monitoring all aspects of service to ensure clients' needs are met.

About Better Together at Home 

Better Together Community Support is a well-established local not-for-profit organisation on Atherton Tablelands, 1.5 kms from Cairns. Our team comprises of highly experienced and skilled professionals in various fields, including social work, community development, psychology, and counselling. We are thrilled to introduce a new role that will actively contribute to the implementation and growth of our innovative Home Care Packages Program under the banner of “Better Together at Home”.

Responsibilities 

  • Clinical Assessment:
    • Conduct clinical assessments for clients using standardized tools within program-specific requirements and identify unmet health requirements.
    • Assess individual health and social needs of clients living at home. 
    • Develop care plans based on mutually agreed goals.
  • Chronic Disease Management:
    • Focus on chronic disease management for target groups.
  • Care Coordination and Partnerships:
    • Coordinate services required for clients, including clinical and non-clinical care.
    • Collaborate with primary case managers (e.g., GPs) to support care partnerships.
    • Monitor patient engagement with care plans and facilitate necessary changes.
  • Health Literacy and Promotion:
    • Provide effective communication to improve health literacy.
    • Ensure clients understand their care plans and follow-up appointments.
    • Promote health and well-being within the community.
  • Allied Health Services and Stakeholder Management:
  • Coordinate allied health services as needed.
  • Collaborate with stakeholders, including GPs, specialist clinics, and health centers.
  • Liaise with representatives from Commonwealth and State Agencies.
  • Quality and Compliance:
  • Adhere to Aged Care Act, contractual obligations, and quality outcomes.
  • Contribute to the review and implementation of Better Together @ Home’s policies and procedures.
  • Ensure compliance with statutory legislation, Australian standards, and industry codes.
  • Supervision and Training:
  • Develop and implement clinical training programs for staff.
  • Participate in performance improvement and continuous quality improvement activities.
  • Provide supervision and guidance to other team members.
  • Guidance and oversight of purchase of medical (assessment) supplies and PPE
  • Develop and implement a purchase system to make sure required medical supplies and PPE are available in a timely matter.
  • Liaise with suppliers to improve the purchase process for these items.

Selection Criteria 

Qualifications 

  • Relevant tertiary qualifications or Australian recognised equivalency. 
  • Current registration with AHPRA as a Registered Nurse. 
  • Police Check, or the ability to acquire a Police Check. 
  • Current Drivers licence 

Experience and Skills 

  • Knowledge of the common physical, psychological and social issues affecting frail older people.  
  • Demonstrated understanding of issues affecting older people from culturally diverse communities. 
  • Demonstrated understanding of the Australian Government funded aged care system and Home Care Packages. 
  • Demonstrated staff supervision skills. 
  • Highly developed interpersonal and communication skills, including ability to undertake complex negotiations and develop networks, relationships and partnerships with internal and external stakeholders. 
  • Proven high level organisational, leadership and motivational skills. 
  • Possession of conflict resolution and negotiation skills. 
  • Ability to meet deadlines on a daily basis, manage competing priorities and tasks and delegate effectively. 
  • An ability to participate in the quality improvement process.
