About the Role
Implement the IUIH network Palliative care program within the ATSICHS catchment, including Jimbelunga Nursing Centre. It is the role of the Care Coordinator Nurse – Palliative Care to support the coordination of care and supports for clients who are on a palliative pathway and their families. This includes effective: client engagement, stakeholder engagement, planning for end of life, and support for holistic care including equipment and medication management.
Duties
1. Provide Care Coordination for clients on a Palliative Care pathway.
2. Provide quality health care.
3. Commit to a high level of quality management for ATSICHS through use of LogiQC and Mobnet pathways.
4. Accurate and timely collection and reporting of episodes of care.
5. Competence in the use of clinical software systems and other technology, the internet and desktop applications e.g. outlook, Mmex, LogiQC, word, spreadsheets and telehealth software.
6. Commit to ongoing continuous quality improvement processes with annual reviews into the efficiency of clinical service provision.
7. Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff.

COVID-19 Requirement
- Immunisation history showing you have received two doses of a Covid -19 vaccine is encouraged.
Essential:
- Minimum Diploma in Nursing and/or Certificate IV (4) in ATSI Primary Health Care Practice
- Full un-restricted AHPRA registration.
- Previous demonstrable experience in relevant nursing field.
- Demonstrated ability to meet targets and performance outcomes.
- Ability to support a continuous improvement model for achieving outcomes.
- Decisive listening, influencing and persuasive skills.
- Current Police Check
- Current Yellow Card
- Meet requirements for vaccinations in Residential Aged Care Facility
Desirable, but not mandatory:
- Minimum two (2) years’ experience in a primary health or aged care setting.
- Previous experience of working with Aboriginal and Torres Strait Islander clients.
- Previous experience in a multidisciplinary health team.
- Previous experience in Palliative care services
