A little about the role

The Care Finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. Care Finders will: 

  • provide specialist and intensive assistance help people to understand and access aged care and connect with other relevant supports in the community; and 
  • specifically target people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the Mitchell and Whittlesea shire
  • Provide assertive outreach while building engagement and rapport with potential clients
  • Guide clients through the assessment process and assist in obtaining Aged Care supports and services needed

To be considered for this rewarding role you will have:

  • Tertiary qualification in Human Services/Social Work or Community Services and or related profession and or previous experience in needs assessment, care planning and/or case management
  • Extensive knowledge of the aged care, health and community services system and regulatory environment
  • Experience in working with older people and their families/carers from a range of backgrounds, and an understanding of the barriers they may experience in accessing aged care services
  • The ability to work collaboratively and autonomously and build effective relationships
  • Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
  • Computer literacy and the ability to effectively manage a client data base
  • High level problem solving, exceptional customer service skills and analytical skills
  • Excellent IT skills, including Microsoft Office suite
  • Knowledge of Carelink + desirable
  • Full Victorian Driver’s Licence
  • COVID Vaccinations including booster

The fine print

  • Successful candidates will be required to undergo a National Police Check, hold a valid WWCC
  • Full driver’s licence and travel between within Mitchell & Whittlesea shire is required (car provided)
