The Role:
The Inner Melbourne Community Hub Case Management team comprises of Key Workers (Case Managers) and Case managers who work closely with single adults who are marginalized, in crisis, homeless or have no bed for the night. The service aims to assist people in securing and maintaining affordable housing by addressing unmet needs via a case management process.
This particular role is a dual role where you will be working across two programs -The Community Connections Program and the Care Finders Program.
The overall aim of the Case Manager position in Community Connections Program is to stabilise the client's crisis, identify and implement seamless pathways to ongoing supports, creating linkages, advocating and supporting clients to access mainstream and specialist services, particularly housing and health services. Care Finder clients receive assistance to better navigate the My Aged Care system with referrals, if relevant to longer term aged care supports. Key to the success in this position is the ability to work from a collaborative, integrated and client centred approach with clients, through the use of ongoing assessment, case planning and review, and referral.

Key Selection Criteria:
- A relevant tertiary qualification (For example Bachelor Degree) (preferred)
- Full Victorian Driver's Licence
- Experience in working with and supporting people who are experiencing homelessness and presenting with multiple needs.
- Experience in case management practice and navigating service systems with and for clients.
- Demonstrated understanding of the range of needs and issues affecting people who are homeless or at risk of homelessness.
- Knowledge of trauma informed, strengths based and person-centred care.
- Highly developed communication and relationship management skills. The ability to maintain collaborative working relationships with a broad range of stakeholders.
- Experience navigating My Aged Care services.
- Experience, knowledge and demonstrated ability in maintaining professional boundaries with clients
- Demonstrated ability to work independently and as a member of a team, taking direction when required.
- Evidenced administrative accuracy and detail, including timely data entry.
