As Care Manager you would have the opportunity to provide high-quality, client-centred care for older adults in their homes across South West Victoria. You'll work closely with clients, families and service providers to develop personalised plans that promote independence and well-being.

In the role you'll:

• Create care plans and coordinate services aligned with client needs and budgets.
• Advocate for clients from diverse backgrounds, ensuring a culturally sensitive approach.
• Develop creative solutions to address complex clinical and social challenges.
• Collaborate with healthcare professionals and service providers to ensure seamless care coordination.
• Promote La Bella Life services within the local community.
• Maintain accurate client records and budgets.
• Contribute to continuous quality improvement initiatives.
• Uphold ethical and professional standards.

What you bring:

• Experience in health care and case management.
• Strong knowledge of the healthcare system, community resources and aged care regulations.
• Excellent communication, interpersonal and problem-solving skills.
• Compassionate and client-centred approach.
• Ability to work independently and collaboratively.
• Commitment to continuous professional development.
• Proficiency in ICT applications and basic budget management.
• Current driver's license and satisfactory police check.
• Reliable vehicle with full comprehensive insurance.
• Must reside in South West Victoria.
