Role Summary
The Care Manager, in collaboration with the Facility Manager and the multidisciplinary team, provides clinical leadership and ensures the smooth operation of the facility. This role is dedicated to delivering resident-focused care services through comprehensive clinical assessments, optimising residents' lifestyles, and maximising Commonwealth and State funding.
Responsibilities and Duties
- Promote and demonstrate a resident-focused approach to care delivery, based on thorough clinical assessments that enhance residents' quality of life and optimise funding.
- Provide supervision and guidance to the care team, ensuring the delivery of high-quality care that aligns with current research and best practice standards.
- Exhibit positive role modelling and leadership in accordance with professional standards, fostering a culture of excellence within the team.
Skills, Attributes, and Experience
- Must be a Registered Nurse with a minimum of 5 years of experience.
- Experience in aged care, preferably within residential and/or community settings.
- Proficiency in managing common medical conditions.
- Understanding and experience with aged care funding by State and Commonwealth.
- Knowledge of quality and risk management systems and Aged Care Quality Standards.
- High-level proficiency in Microsoft Office and Excel.
- Excellent organizational abilities to manage responsibilities and priorities effectively, and assist others in doing the same.
- Exceptional verbal and written communication skills.
- Ability to develop staff, foster teamwork, and work effectively as a team member while providing leadership.
