The role: Based at our stunning historic home in Glenelg, and in accordance with our vision, and cultural elements of ACH Group, the Care Manager is responsible for providing and supporting quality customer care, that meets the needs of residents and ensuring that a good life is made possible via nursing interventions and proactive clinical care to promote good health, optimum independence and decision making for residents.
What you'll do:
- Complete comprehensive and accurate nursing assessments for residents
- Ensuring the care plan is collaborative, holistic, and person-centred to support the needs of the resident.
- Promote the utmost wellbeing of residents.
- Consult and communicate across a multi-disciplinary team internally and externally to identify resident needs, plan interventions and review outcomes of care.
- Promote communication between team, family, and resident.
- Have a current registration with AHPRA as a Registered Nurse
- Minimum 2 years post-graduate nursing experience and experience working within the aged care environment advantageous.
- Ability to maintain appropriate standards and systems to the highest quality.
- Knowledge and understanding of the Aged Care Quality & Safety Standards, legislative and compliance requirements, and funding model.
- Excellent attention to detail.
- Current COVID-19 vaccinations (3)
- A satisfactory Police Check for working with vulnerable groups less than 12 months at commencement.
