An outstanding opportunity has presented itself for an experienced Care Manager to join our friendly West Beach Residential Care Team.
About the role
As a Care Manager with Southern Cross Care, you will work with the Residential Services Manager to implement a healthy and active ageing service delivery model, which ensures resident choice, independence, and quality of life are maintained at the highest level.
You will work with your colleagues and residents to develop and implement goal-oriented care plans. You will lead, support and mentor the care team to ensure optimal resident outcomes, and will lead the early intervention working group to develop recovery and fitness pathways for residents.

About you
The successful candidate will have knowledge and understanding of Aged Care Funding Instruments and will be passionate about providing high-quality person-centred care. You will possess effective leadership skills and aged care experience including the ability to work at an advanced level of nursing practice in the care and management of older people. You will also have exceptional organisational skills including the ability to manage a range of priorities.
With a Bachelor of Nursing or equivalent, current AHPRA registration and an impressive reputation for person-centred service in aged care, you will also demonstrate high-level listening, communication, team building and time management skills.
Candidates with Care Manager experience in an Aged Care or Health Care setting will be highly regarded. Additionally, previous experience in a Memory Support Unit (MSU) would be an advantage.
