Your New Role
In this role you will collaborate with various stakeholders to deliver a high standard of care and support to consumers including leading a team of care workers who will deliver services. This role will be based at Chermside with the option to work from home. In this role your day to day work will be varied with responsibilities including but not limited to:
- Actively engage with consumers, their families and other relevant stakeholders to ensure the best outcomes and ongoing alignment with consumer goals
- Develop care plans
- Coordinate care services to consumers
- Ensure services are in line with the assessed care needs, agreed goals and remain within budgeted parameters
- Consult and refer to clinical staff where necessary
- Willingness to use your own comprehensively insured vehicle and drive in the course of work
What you'll bring to the role
- Experience and knowledge in managing and supporting teams
- Prior aged care and HCP experience
- Ability to coordinate care services and case manage with minimal supervision
- Understand the principles of Consumer Directed Care
- Highly effective interpersonal, oral and written communication skills, including the ability to develop and maintain effective relationships with a wide range of stakeholders
- Highly effective organisational skills, particularly in prioritisation and time management
- Sound computer skills with the Microsoft Office suite of applications and client databases
