The Case Coordinator Role
The Case Coordinator will provide a comprehensive Home Care Package Program including identification of needs, assessment, implement, coordination, maintenance and evaluation and review of home and community-based programs within the parameters and criteria of the Eyre & Far North Local Health Network (EFNLHN).
As a member of a multi-disciplinary team, the Community Care Coordinator assists in the development, implementation, coordination and evaluation of community-based ageing and disability programs to support those who wish to remain living in their own home with some assistance.
