About the role:
We are seeking a dedicated Clinical Care Case Manager to work as part of Call on Clare’s Clinical team. As a Case Manager your role will be to coordinate and provide care that is timely, tailored and client centred. At Call on Clare our Case Manager facilitates multiple care aspects encompassing clinical and daily living care coordination, information sharing with external medical organisations, advocacy and financial options so our clients can make informed decisions and retain independence and autonomy.
Case Managers help their patients understand their options concerning the specific situation they are dealing with at the time. They are liaisons between patients and their treatment or care options.
Your strong attention to detail, demonstrated active listening and empathy skills alongside your clinical expertise with Home Care Package and NDIS administration knowledge will be essential in providing care to our clients and fostering a strong sense of collaboration amongst the clinical and wider teams at Call on Clare.
Key Responsibilities:
Client Assessment:
- Conduct comprehensive assessments of clients' physical, emotional, and social needs.
- Collaborate with clients, families, and healthcare professionals to gather relevant information for the assessment process.
Care Planning:
- Develop and implement client-centred care plans based on assessments taken from clients and family members for both clinical and social care support.
- Regularly review and update care plans to reflect changes in clients' needs and preferences.
Service Coordination:
- Coordinate and arrange a variety of in-home support services, including personal care, social care, nursing care, therapy, and allied health services.
- Liaise with external service providers to ensure seamless delivery of care.
Client Advocacy:
- Act as an advocate for clients, ensuring their rights, preferences, and choices are respected and upheld.
- Communicate with relevant stakeholders to address any concerns or challenges faced by clients.
Monitoring and Evaluation:
- Regularly monitor clients' progress and adjust care plans accordingly.
- Conduct regular evaluations to assess the effectiveness of the care provided and make recommendations for improvements.
Documentation:
- Maintain accurate and up-to-date records of client assessments, care plans, and ongoing communications.
- Prepare reports as required for internal and external stakeholders.
- Experience with Home Care Packages and Government agencies.
Qualifications and Experience:
- Registered Nurse, or a related field.
- Proven working experience in case management, including as a Nurse, Medical, Mental Health Care Manager or a related job.
- Previous experience in case management within the aged care sector.
- Knowledge of the Australian aged care system and portals.
- Excellent communication, interpersonal, and problem-solving skills.
- Empathy, patience, and a genuine commitment to improving the lives of aged care clients.
- Experience with AlayaCare or other PMS desirable.
